Cano Health Awarded NCQA Patient-Centered Medical Home Recognition

MIAMI – Cano Health, LLC has been awarded Patient-Centered Medical Home (PCMH) recognition by the National Committee for Quality Assurance (NCQA).

The Patient Centered Medical Home is a model of care that puts patients at the forefront. Research shows that PCMHs improve quality and the patient experience, increase clinical staff satisfaction, and reduce health care costs. Practices that earn this recognition have made commitments to continuous quality improvement and patient-centered approaches to care.

Earning NCQA’s PCMH Recognition

NCQA’s PCMH recognition program is the most widely adopted PCMH evaluation program in the United States. To receive this recognition, Cano Health was required to complete criteria in six concept areas:

  1. Team-based care and practice organization
  2. Knowing and managing patients
  3. Patient-centered access and continuity
  4. Care management and support
  5. Care coordination and care transitions
  6. Performance measurement and quality improvement

Cano Health’s PCMH recognition started May 2020 and is effective for one year. To retain the PCMH recognition beyond May 2021, Cano Health is required on an annual basis to submit to NCQA evidence of ongoing quality improvement.
