Welcome to Cano Health.

We are transforming patient care in America by delivering superior primary care services for seniors and their communities. We have the privilege of serving more patients than ever before because we have successfully executed our model and continue to produce better clinical outcomes at lower costs.

Why Cano Health was founded is at the heart of what we do every day: we want to make healthcare easier, while treating our patients like family.

Our mission is simple: To improve patient health by delivering superior primary care medical services, while forging a life-long bond with our members.

Our vision is clear: To become the national leader in primary care by improving the health, wellness and quality of life of the communities we serve, while reducing health care costs.

We are well on our way. Our mission to transform healthcare has changed our communities for the better. And the reason is simple: We put our members first.

We are entering a new era in the medical industry where value-based care is delivered through an integrated model – and the integrated model itself becomes a powerful differentiator. Some may feel that “care will be a commodity,” but the risk management, individualization, care coordination, and analytics distinguish Cano, as these produce the best health care outcomes and results.

The challenges are plain to see. The American Diabetes Association estimated that total medical costs and lost work and wages for people with diabetes annually is $245 billion. That’s diabetes alone. By forging a deeper, holistic relationship with our members, we not only help improve lives – we also help reduce the tremendous cost burden our nation faces.
