Cano Health Achieves Age-Friendly Health Systems Distinction

Miami, FL – (September 8, 2022) – Cano Health, a leading value-based primary care provider and population health company, announced that it has achieved Level 2 (Committed to Care Excellence) recognition in 56 of its clinical sites as part of its participation in the Institute for Healthcare Improvement (IHI) Age-Friendly Health Systems initiative.

The Level 2 milestone recognizes clinical care settings that reliably provide high-quality, age-friendly care according to four elements, known as the 4Ms, for older adults. The 4Ms represent: 1-What Matters, 2-Medications, 3-Mentation, and 4-Mobility and act as a framework to make care for older adults manageable. They also identify core issues that should drive decision-making in caring for older adults. To achieve Level 2, the Age-Friendly Health System must have achieved a Level 1 recognition and submitted at least three months of counts of the number of older adults that have received care that included all 4Ms.

Cano Health achieved accreditation as a national Age-Friendly Health Systems participant across its network of primary care clinics in January 2021, recognizing its work to improve healthcare for older adults, tailor care to avoid hospital readmissions, and achieve better overall patient outcomes.

The Age-Friendly Health Systems initiative was launched by The John A. Hartford Foundation and the Institute for Healthcare Improvement, in partnership with the American Hospital Association and the Catholic Health Association of the United States, to help hospitals and other care settings implement a set of evidence-based interventions specifically designed to improve care for older adults.
